ALERT:  AFLAC New Business in Utah Ends February 12th 2010!  Aflac  will only accept new business for its individual product lines in Utah until 2/12/10. These plans are currently the number one plans used for Maternity in Utah. Although we still have other plans, this is a major blow to the maternity market it Utah. This means we have to have the applications to our office by 2/12/10. Spread the word so others don't get left out. We can help you get this product before it is gone!

1.  AFLAC 
This option ends 2/12/10!  Spread the word!
$102.09/month (Can cover wife alone, cost is per adult, cost is more if you want to add children)
There is a 10 month waiting period from the effective date before this can be used for maternity
(ie must deliver in month 11 or beyond. Deliveries within the first 10 month will be denied!)

This price consists of two policies that add up to the $102.09 premium.
Both policies are necessary to get the benefit described.

Benefit is $2,000 for admit in hospital + $550/day 
+ $15 for up to 3 office visits per calendar year
So $3,145 for a 2 day stay, and $4,345 for a 4 day C-Section

2.  UNL - United National Life
Apx *$97
/month (Must cover both spouses)
No 10 month waiting period, just can't be pregnant when the policy is put into effect.
The Effective Date is the date of your signature!
EXCLUDES NORMAL DELIVERY IN UTAH!  Great if you have a C-Section!
$725/day for Hospital Confinement Benefit (so 2 days = $1,450, 4 days=$2,900)
$75/Dr Visit benefit up to 10 per calendar year (must submit a bill for each visit, no global billing)

.  AFLAC - Specified Health Event Protection Policy
$23.40/month (Can cover wife alone, cost is per adult, cost is more if you want to add children)
You can add the child at birth. This policy would then pay for health issues such as an ICU stay
Although it doesn't have the 10 month waiting period like Option #1, please understand that
you cannot be pregnant when you apply for this policy!

Benefit under this policy:
$700 for admit in an ICU for Days 1-7
$1,200 for admit in an ICU for Days 8-15
No Benefits after day 15 for ICU stay


Aflac also pays $350/day in the case of Coma, Stroke, Paralysis, Heart Attack,
Kidney Failure, 3rd Degree Burns, Organ Transplant and more.

.  APL - American Public Life
$161.88/month (Or $188.80 if applicant is over 34, and $266.90 if over 44)
If married, you must cover Husband AND Wife at the premium above (includes both spouses in rate above).

Note: You c
annot have this option in force if any other Indemnity plan (like Aflac) is in force first.
So, you must purchase this plan for a 1st or 15th start date

Common Questions and Answers:

Q)  Why can't I go directly through the companies instead of an Agent like you?
A)  The companies do not have a marketing department. They rely on Agents like us to take your phone calls, answer you questions and process your applications. If you tried to call the companies directly they would send you to one of us to take care of you. You don't pay us, the companies compensate us by paying a commission based on the number of applications we send in.

Q)  How does the Aflac "Specified Health Event Protection Policy" work?
A)  This is a policy that we highly recommend since it will cover both mother and/or newborn children if they have major complications that include a stay in the ICU. You must add the child to the mother's plan within 30 days of birth in order for the newborn to be covered and pay more for the single parent option. But the huge payout is well worth having this option as part of your insurance portfolio.  There is a 30 day waiting period before benefits can be paid and you must not be pregnant on the effective date of the plan.

Q)  When will my policies be in force (ie What will be the Effective Date)?
A)  Option #1 (Aflac) will start within a few business days of when they get the paperwork. That is when the 10 month waiting period starts for Aflac.  You can also ask for a future date. Option #2 (UNL) will start within a few business days of when you apply. There is no "10 month" waiting period on Option #3 but you can't be pregnant on your effective date. Remember Option #2 will only cover you in the event of a C-Section (excludes normal delivery in Utah)


Q)  What is up with the 10 month rule for Maternity?
A)  Although the companies cannot stop you from purchasing their policies for the purpose of using it for maternity, they can put limitations for pregnancy. For example, Aflac (Option #1 above) puts a 10 month exclusion on their plans.  This means that you must deliver in month 11 or beyond from the effective date of your policies. But remember that these policies are for any eligible hospitalization - They just so happen to cover maternity after 10 months.  The UNL plan (Option #2 above) does not have any waiting period for maternity, but it excludes normal delivery (only pays on C-Sections) and you can't be pregnant on your effective date.

Q)  Can anyone give me a lower price?
A)  No. We all work off the same rate sheets so you can't get a "sweetheart" deal by shopping around. I really wish I could get a lower price, but it doesn't work that way. We do more supplemental maternity plans like this than any other Agency in Utah. Some new Agents try the business but don't get enough volume to maintain their contracts with the company. We have been doing this since 1997 and will be there for you in the future to answer your questions, etc.

Q) Do I pay you, or the company? 
A)  You pay all money to the company. We collect nothing but a VOID check to send to the company to set up payments.  Both of the companies will take a credit card payment also. Remember that you are contracted with the company, not me.

Q) When will the first withdrawal happen?

A)  Generally the money will come out within the next week or so for the first month's premium.

Q) Can I cancel these plans anytime I want to?
A)  We of course encourage you to "build these plans into your long term insurance portfolio. But if you decide to cancel  be very careful to make sure the policy continues to be in force through any claims you may have.  But yes, you can cancel anytime month to month. There is no obligation on your part to continue any longer than you would like to continue.

Q)  What will I get from the companies to to give evidence of my policies?
A)  You should receive paperwork from each of the companies within a week or two of your applications.

Q)  How would I file a claim?
A)  If you or a covered family member has a claim (hospitalized for at least 24 hours) you can fill out a claim form and fax that claim form to the company with a copy of your hospital bill. See above summary of companies for claim form links and fax numbers. 

Q)  Can I pay by Credit Card?
A)  Options #1 (Aflac) allows credit card payments and does not charge extra for Credit Card payments. Option #2 (UNL) does not accept credit card payments.

Q)  Are there any start up fees, Application Fees or other charges?
A)  Yes.  Option #2 (UNL) has a one time non-refundable $20 application fee. Option #1 (Aflac) has no application fee and no extra charge to use a credit card for payment.
